Auckland Bridging Programme

Established in 1997, the Auckland Bridging Programme is a multimodal service incorporating Residential Care, Treatment Foster Care and Multi Systemic Therapy. This programme provides individual therapeutic planned interventions for young people who live in the Greater Auckland Area, are diagnosed with Severe Conduct Disorder and are at risk of poor life outcomes

Lower North Severe Conduct Disorder Programme

This is a 3 stage collaborative programme with CCDHB at Epuni, of which Youth Horizons provides the stage 3, community reintegration component. The service uses MST, Community Mentoring and Treatment Foster Care to reintegrate young people coming out of Epuni Residential Units. Philosophies central to the service include taking all steps to remove barriers to reintegration, involving whanau, using a multi-system orientation and focusing on biculturalism.

Hamilton Specialist Family Home

Established in 2001, this Specialist Family Home caters for 5 young people with significant behavioural problems, including offending, who are at risk of poor life outcomes. The service is provided under the Child, Youth and Family Youth Services Strategy for male rangatahi and their whanau with the aim of reducing persistent re-offending and improving rehabilitative outcomes for high risk youth.

Bay of Plenty Respite Care:

Established in 2007, this service provides 3 levels of Crisis and Planned respite care for those at risk of admission to an inpatient mental health acute unit, with a view of reducing harm caused by inpatient admissions and supporting the preservation of natural families.

Ka Awatea

An Auckland based programme established in 2003 for young people leaving the care of Child Youth and Family for whom the permanency goal is independence. The service provides strengths based, one to one, support by Kai Atawhai (Personal Advisors) as young people transition to independence. This includes practical support with personal documentation, drivers’ licenses, accommodation and employment alongside positive role modeling in communication and relationship management.

Lakes Multi Systemic Therapy:

A service established in 2006 for children and adolescents with mental health issues and with active involvement with CYF. Services are delivered in the young person’s home and aim to provide integrated family supports through utilising structured Multi Systemic Therapy interventions.

Lighthouse Programme:

Established in 2007 as a full Joint Venture with Youthlink Family Trust, this programme provides short-term placement and respite for youth in the Auckland Region. It involves an intensively supervised, non-secure facility for both boys and girls, generally with histories including abuse, neglect, offending, alcohol and drug issues, family violence and mental health concerns.

Waikato Respite and Emergency Specialised Foster Care

Established in 2004, this service aims to increase the likelihood that young people with serious mental health problems can return to their home environment, by providing timely support to families, in collaboration with mental health services. Youth Horizons trains, recruits and provides 24 hour, 7 day a week support to the team of Respite Carers and Foster Parents throughout the Waikato.

Specialist Caregiver Services:

This service is part of the CYF Youth Services Strategy and provides community based Treatment Foster Care placements for young people with extremely challenging behaviour. The fostercare placement is supported by flexible individual interventions and behaviour management strategies which aim to reintegrate the young person into the community and reconnect them to family and pro-social peers.

Te Hurihanga:

This is a 3 year pilot programme involving the operation of a rehabilitative and re-integrative intervention programme for recidivist young male offenders (aged 14 – 17) in the Hamilton/Waikato area, with a view to prevent otherwise inevitable periods of imprisonment. The service is a multiphase, residential and community therapeutic programme designed to reduce re-offending and hold young people accountable.

Waitakere Youth Transition Service:

Opened in 2005, this service aims to ensure all Waitakere school leavers have access to training, further education, employment or other activities that will contribute to their economic independence and wellbeing. In addition the service provides intensive customized support to those young people who have multiple risk factors and require additional assistance to successfully transition into productive activity.

Waikato Intensive Clinical Services:

This programme provides Intensive Clinical Intervention services to children, adolescents and youth with serious mental health problems and who have active involvement with Child Youth and Family. Services are delivered in the young person’s home and aim to provide integrated family supports through utilizing specialist respite foster care and structured Multi Systemic Therapy interventions.
